Letter of Agency Form Instructions

1. Click here for form.
2. Enter your name in the AUTHORIZATION GIVEN BY section.
3. List Name as appears on your local phone bill.
4. List Social Security Number for residential account or FIN business accounts.
5. List Estimated Monthly Usage.
6. List Billing Address and Physical Address if different from Billing Address.
7. List Telephone Number(s) To Switch To UniTel Long Distance Service.
8. Select if you would like UniTel to also be your long distance carrier for IntraLATA  ("local long distance") calls. (UniTel bills intraLATA calls at the instate rate.)
9. Indicate the number of calling cards tht you desire. (Calling cards and PINs are sent by mail after your account is established.)
10. Complete the Toll Free Service section if you desire toll free service for either an existing toll free number or a new toll free number. If you are transferring an existing number, please also complete a Responsible Organization Transfer form (see below).
11. Print out application.
12. Include your Signature and the Date. (Applications without signatures cannot be processed.)
