To apply off-line for UniTel toll free ONLY service for a new toll free number, please submit a Toll Free Confirmation form.

Toll Free Confirmation Form Instructions

  1. Click here for form.
  2. Enter Legal Company Name and DBA  Name. (If for a residential account, list your name.)
  3. List Ring To Number(s). One toll free number will be assigned to each ring to number listed.
  4. Complete the Customer Information section.
  5. Print your Name and Title.
  6. List your Social Security Number of Federal Identification Number (FIN).
  7. Print out application.
  8. Include your Signature and the Date. (Applications without signatures cannot be processed.)
  9. Fax to 877-511-3207.
