Use your credit card to automatically pay your UniTel invoice each month. Simply submit a AutoPay Credit Card Authorization form to take advantage of the convenience of automatic payment. You will still receive a detail mailed invoice.

AutoPay Credit Card Authorization Form Instructions

  1. Click here for form.
  2. Enter Date.
  3. List Contact Phone Number.
  4. List your UniTel Customer Account Number.
  5. List your Name As It Appears On Credit Card.
  6. List your mailing Address for your credit card invoice.
  7. List your Credit Card Number.
  8. Enter Expiration Date and select Card  Type.
  9. Check YES to authorize the monthly charging of your credit card.
  10. Enter your name in the Customer's Name  field.
  11. Print out application.
  12. Include your Signature.
  13. Fax to 877-511-3207.
